To provide direct nursing care to patients in Care Chexx LLC’s recuperative care facility for homeless adult males and females; to observe and report patients' physical conditions; to assist in collecting and recording data for patient assessments; and to perform related duties as assigned.

The setting emphasizes stability, dignity, and recovery. Clients are housed through Care Chexx’s partnership with a local hotel, allowing everyone to stay in a private room. This provides a quiet, clean, and secure space where clients can rest, heal, and regain strength—something that is often not possible in traditional shelter environments.
The program serves both adult males and females and is structured around short-term recovery. Clients are typically approved for a 21-day stay, with the ability to extend up to 60 days if medically necessary and authorized. To ensure the environment remains appropriate for recuperative care, clients must be ambulatory and capable of independently managing basic daily needs, including toileting.

Daily responsibilities involve consistent rounding on clients to observe their overall condition and recovery progress. During rounding, staff take vital signs as needed and assess for any changes in physical or behavioral health. This includes monitoring for signs of improvement or decline, checking wound status when applicable, and ensuring the client remains ambulatory and able to perform basic activities of daily living. Observations are not limited to clinical indicators but also include general wellness, engagement, and any barriers to recovery.
Client assessments are ongoing and informal as well as structured. Staff use these interactions to reinforce healthcare education, provide medication reminders, and ensure clients understand their care instructions. Any concerns identified during rounding are addressed promptly and escalated when necessary to appropriate providers or care coordinators.
Documentation is a critical and continuous function. Staff record all observations, vitals, assessments, and services provided in a timely and accurate manner. Minimum Qualifications
Education: Two-year associate degree in nursing. Bachelor’s degree or higher in nursing is preferred.
Experience: None required.
Certifications/Licensure: Must hold a valid Registered Nurse license in the State of Minnesota or a permit to practice professional nursing as issued by the Minnesota Board of Nursing.
Supervisory/Managerial Responsibility:
- None but may provide guidance to LPNs, medical assistants, and other support staff.
- Knowledge of nursing practices, pharmacology, and anatomy.
- Skill in emergency response, patient assessment, and treatment.
- Ability to build effective relationships with patients, families, and team members.
