Basic Function

To provide direct nursing care to patients in Care Chexx LLC’s recuperative care facility for homeless adult males and females; to observe and report patients' physical conditions; to assist in collecting and recording data for patient assessments; and to perform related duties as assigned.

Recuperative care at Care Chexx is designed as a structured, supportive recovery environment that bridges the gap between hospitalization and independent living. It operates similarly to home health care but is specifically tailored for adults experiencing homelessness who no longer require hospital-level care, yet are not well enough to safely recover on the streets or in unstable conditions.

The setting emphasizes stability, dignity, and recovery. Clients are housed through Care Chexx’s partnership with a local hotel, allowing everyone to stay in a private room. This provides a quiet, clean, and secure space where clients can rest, heal, and regain strength—something that is often not possible in traditional shelter environments.

The program serves both adult males and females and is structured around short-term recovery. Clients are typically approved for a 21-day stay, with the ability to extend up to 60 days if medically necessary and authorized. To ensure the environment remains appropriate for recuperative care, clients must be ambulatory and capable of independently managing basic daily needs, including toileting.

Within this setting, clients receive supportive services aimed at promoting full recovery and stabilization. The environment is not clinical like a hospital, but it is more structured and supportive than standard housing—striking a balance that allows individuals to heal while preparing for their next step, whether that is permanent housing, continued care, or reintegration into the community.

Typical Workday

Work at Care Chexx Recuperative Care centers on maintaining a clear, ongoing understanding of each client’s medical needs while supporting their recovery in a stable environment, consistent with expectations from the Minnesota Department of Human Services.

Staff are expected to continuously familiarize themselves with each client’s reason for admission into recuperative care. This includes reviewing referral information, hospital discharge instructions, diagnosis codes, and any provider recommendations. Understanding why the client no longer requires hospital-level care—but still needs supportive oversight—guides all interactions and ensures services remain appropriate and compliant.

Daily responsibilities involve consistent rounding on clients to observe their overall condition and recovery progress. During rounding, staff take vital signs as needed and assess for any changes in physical or behavioral health. This includes monitoring for signs of improvement or decline, checking wound status when applicable, and ensuring the client remains ambulatory and able to perform basic activities of daily living. Observations are not limited to clinical indicators but also include general wellness, engagement, and any barriers to recovery.

Client assessments are ongoing and informal as well as structured. Staff use these interactions to reinforce healthcare education, provide medication reminders, and ensure clients understand their care instructions. Any concerns identified during rounding are addressed promptly and escalated when necessary to appropriate providers or care coordinators.

Documentation is a critical and continuous function. Staff record all observations, vitals, assessments, and services provided in a timely and accurate manner. Notes reflect the client’s current condition, progress toward recovery, and any notable changes. Documentation also includes tracking and reminding clients of upcoming medical appointments, ensuring they are prepared and able to attend.

Care coordination is integrated into daily responsibilities. Staff schedule and confirm follow-up appointments, arrange transportation, and maintain communication with external providers. After appointments occur, staff update client records with new instructions, diagnoses, or treatment changes, ensuring that care plans remain current and aligned with provider’s recommendations.

Overall, duties at Care Chexx focus on maintaining awareness of each client’s medical purpose for being in recuperative care, actively monitoring their condition through rounding and assessment, and ensuring all observations, appointments, and follow-up information are thoroughly documented and acted upon to support safe and effective recovery.

Minimum Qualifications

Education: Two-year associate degree in nursing. Bachelor’s degree or higher in nursing is preferred.

Experience: None required.

Certifications/Licensure: Must hold a valid Registered Nurse license in the State of Minnesota or a permit to practice professional nursing as issued by the Minnesota Board of Nursing.

Supervisory/Managerial Responsibility:

  • None but may provide guidance to LPNs, medical assistants, and other support staff.
  • Knowledge of nursing practices, pharmacology, and anatomy.
  • Skill in emergency response, patient assessment, and treatment.
  • Ability to build effective relationships with patients, families, and team members.
  • Competence in using medical equipment and interpreting laboratory results.
  • Ability to handle stress and manage emergencies effectively.
  • Strong organizational and interdisciplinary collaboration skills.

Pay: $45.78 - $51.55 per hour
