Position Summary
The Licensed Practical Nurse) is responsible for providing and coordinating comprehensive nursing care to individuals residing at Care Centers of Nassau. This role ensures the delivery of high-quality medical care in compliance with physician orders, state regulations, and facility policies while promoting the health, safety, and well-being of individuals served.
Essential Duties and Responsibilities
- Provide direct nursing care including assessments, treatments, and medication administration in accordance with physician orders and care plans.
- Monitor and document residents’ health status, vital signs, and changes in condition.
- Administer medications and treatments accurately and in compliance with all regulations and facility policies.
- Coordinate with physicians, interdisciplinary teams, and healthcare providers regarding resident care and treatment plans.
- Develop, implement, and update nursing care plans based on resident needs.
- Respond to medical emergencies and provide appropriate interventions as needed.
- Ensure proper documentation in medical records in a timely and accurate manner.
- Supervise and provide guidance to direct support professionals (DSPs) and other support staff as needed.
- Ensure compliance with infection control, OSHA, HIPAA, and all applicable federal and state regulations.
- Monitor medication storage, administration records, and controlled substances in accordance with policy.
- Participate in care plan meetings, staff meetings, and interdisciplinary team discussions.
- Educate staff and residents as appropriate regarding health conditions and care needs.
- Maintain professionalism, accountability, and ethical nursing practice at all times.
- Attend required trainings, in-services, and continuing education programs.
- Perform other duties as assigned.
Qualifications
Required Qualifications
- Current and valid Registered Nurse (RN) license in the state of Florida.
- Graduate of an accredited nursing program (Associate or Bachelor’s degree in Nursing).
- Basic Life Support (BLS) certification required.
- Knowledge of nursing practices, medical terminology, and patient care standards.
- Ability to document clearly and communicate effectively with staff, residents, and healthcare providers.
Preferred Qualifications
- Experience in intellectual/developmental disabilities (IDD), long-term care, or intermediate care facilities preferred.
- Experience in medication administration and care coordination in a clinical setting preferred.
Physical Requirements amp; Working Conditions
- Frequent standing, walking, bending, lifting, and moving throughout the shift.
- Ability to lift up to 30 pounds.
- Exposure to communicable diseases, bodily fluids, and medical environments.
- Ability to respond quickly to emergencies and high-stress situations.
- Must maintain professionalism and appropriate clinical appearance at all times.
- May be required to work weekends, holidays, and varied shifts.
