The ideal candidate will possess extensive knowledge in wound management, advanced nursing skills, and experience working in diverse clinical settings in long-term care facility. This role involves providing expert wound assessment, treatment planning, and patient education to promote healing and improve patient outcomes. The Wound Care Nurse will collaborate with multidisciplinary teams and utilize various EMR/EHR systems to ensure comprehensive documentation and quality care delivery.
Responsibilities
- Conduct thorough wound assessments, including evaluating wound size, depth, exudate, and infection signs
- Develop and implement individualized wound care plans based on best practices and patient needs
- Administer advanced treatments such as dressing changes and assessments of wounds.
- Educate patients and families on wound care techniques, prevention strategies, and post-discharge instructions
- Document all patient interactions accurately within EMR/EHR systems.
- Collaborate with physicians, physical therapists, dietitians, and other healthcare professionals to optimize patient recovery
- Ensure adherence to HIPAA regulations and infection control standards across all procedures
- Participate in quality improvement initiatives aligned with NCQA standards and long term care protocols.
Skills
- Strong knowledge of medical terminology, anatomy (including physiology), and diagnostic evaluation techniques
- Skilled in advanced nursing procedures such as sterile technique, venipuncture (phlebotomy), catheterization (including ostomy), tube feeding management, and aseptic technique
- Experience administering injections and performing physical examinations with a focus on infection control and aseptic practices
- Ability to perform complex assessments involving vital signs, laboratory specimen collection/processing, and medical documentation
- Familiarity with pain management protocols including acute pain management strategies for complex wounds.
- Knowledge of ICD coding (ICD-9 & ICD-10), CPT coding for procedures performed
- Excellent patient care skills with a compassionate approach working with diverse populations including pediatrics (neonatal & toddler care), geriatrics (dementia & Alzheimer's care), patients with disabilities or developmental delays
