Job Title: Integrated Care Nurse PractitionerLocation: must live within 10 miles of Scottsdale, AZSalary: $120,000 - $150,000 per yearJob Overview: The Advanced Practice Provider delivers personalized, value-based medical care to patients living with chronic kidney disease and end-stage renal disease. This is a field-based role providing in-home and virtual comprehensive assessments, care coordination, and preventative management. The APP works closely with field nurses, community health workers, and physicians to improve outcomes, reduce avoidable hospitalizations, and enhance quality of life.This is not an office-based position.
