Job Title: Care Coordinator
Location: New Castle County (Delaware)
Salary: $30-$35/Hr.
Job Summary:
Care at Home of Delaware (CAHDE) is seeking an experienced Care Coordinator to join our growing team. This role reports to the Lead Care Coordinator. As a Care Coordinator, you will serve in a key role for the organization as a liaison for clients and administration. The ideal candidate will have a passion for helping seniors and people with disabilities live independently in their homes.
Responsibilities:
- Admit new clients in the New Castle County to the Personal Assistance Service Agency and perform recertifications for clients in your caseload in compliance with Delaware state regulations and CAHDE policies
- Develop and revise service plans to meet the individual needs of each client.
- Coordinate with the Lead Care Coordinator to ensure client charts are maintained and in compliance with state of Delaware regulations and CAHDE policies.
- Report incidents, accidents, deaths, or changes in condition to the Lead Care Coordinator.
- Travel to and from client's location for in-person visits.
- Ad hoc projects as needed.
Qualifications:
- Associate degree minimum required
- 2 years of home care experience required
- Works well in fast-paced environments
- Strong assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation
