*Overview* We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have a strong background in nursing, case management, and experience in various healthcare settings such as ICU, geriatrics, or hospital medicine. *Duties* - Conduct comprehensive assessments of patients to evaluate their medical needs and develop individualized care plans. - Monitor vital signs and other health indicators to ensure patient safety and well-being. - Collaborate with interdisciplinary teams to facilitate effective communication and coordination of care. - Implement discharge planning processes to ensure smooth transitions from hospital to home or other care settings. - Provide education and support to patients and their families regarding treatment options, resources, and self-care strategies. - Perform diagnostic evaluations as needed to assess patient conditions and adjust care plans accordingly. - Advocate for patients within the managed care system to secure necessary services and resources. *Requirements* - Current Registered Nurse (RN) license in the state of practice. - Proven experience in case management, preferably in a hospital or managed care environment. - Strong knowledge of discharge planning processes and best practices. - Familiarity with vital signs monitoring and assessment techniques. - Experience working with geriatric populations or in ICU settings is highly desirable. - Excellent communication skills, both verbal and written, with the ability to work collaboratively in a team-oriented environment. - Strong organizational skills with the ability to manage multiple cases effectively.
