Additionally, the role involves educating residents and their families about health management and advocating for residents' rights and well-being. Ultimately, the LPN contributes to a safe, supportive, and healing environment that promotes positive health outcomes for all residents.
Job Responsibilities
- Provide direct resident care including medication administration, wound care, and telemetry monitoring during weekend shifts.
- Assess and document patient conditions, reporting any changes promptly to the healthcare team.
- Educate residents and their families on health maintenance, treatment plans, and post-discharge care instructions.
- Advocate for residents needs and preferences, ensuring their rights and dignity are respected at all times.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Current and valid Licensed Practical Nurse (LPN) license in the state of Indiana.
- Basic Life Support (BLS) certification.
- Experience in direct resident care, including medication administration and wound care.
- Ability to work independently and as part of a healthcare team during weekend shifts.
- Previous work experience in a long-term care or rehabilitation setting.
- Strong communication and resident advocacy skills.
- Familiarity with electronic health record (EHR) systems.
