As a Licensed Practical Nurse (LPN) in our Virtual Care Center, you will serve as the key point of contact for patients and staff by providing exceptional customer service and high-quality patient care. You will be responsible for managing high-volume triage calls, assessing urgent patient needs, and supporting various clinical and administrative functions that ensure efficient and compassionate care delivery.
How You’ll Drive Our Mission Forward:
- Triage incoming calls to determine urgency and coordinate timely medical response
- Accurately document patient complaints, symptoms, and actions taken in the electronic medical record (EMR)
- Support risk management by maintaining precise clinical documentation
- Communicate directly with providers about patient concerns and clinical status
- Provide education to patients and families, ensuring understanding of care plans and instructions
- Support the overall flow of clinic operations through scheduling, medication refills, and patient messaging
- High school diploma required
- Active LPN license issued by the Board of Nursing
- Basic Life Support (BLS) certification required
- Advanced Cardiovascular Life Support (ACLS) certification optional
- Minimum of 2 years of LPN experience, or 1 year of cardiology LPN experience
- Excellent communication and customer service skills
- Familiarity with electronic medical record systems and clinical documentation
