Job Summary
We are seeking a compassionate and dedicated PM Caregiver/Med Passer to join our team in providing exceptional care to residents in a supportive and nurturing environment. Duties
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Administer medications accurately and safely following prescribed schedules and protocols
- Monitor vital signs such as blood pressure, pulse, temperature, and respiration rates to track residents’ health status
- Provide specialized care for residents with dementia or Alzheimer's disease, implementing memory care strategies and behavior management techniques
- Support residents during meal preparation, feeding, and hydration to promote proper nutrition and hydration
- Use Electronic Medical Record (EMR) systems to document care activities, medication administration, and resident responses
- Operate equipment such as Hoyer lifts or other assistive devices to ensure safe transfers and mobility assistance
- Maintain a clean and safe environment by performing light cleaning duties in resident areas and adhering to infection control standards
- Support residents’ social engagement by encouraging participation in activities and providing emotional support
- Collaborate with healthcare professionals, social workers, and family members to develop comprehensive care plans tailored to individual needs
- Drive residents when necessary for appointments or community outings while maintaining safety protocols
Experience
Candidates should possess prior experience in long-term care settings such as nursing homes, assisted living facilities, or personal care homes. Experience working with individuals with developmental disabilities or those requiring memory care is highly valued. A background in caregiving roles that involve assisting with ADLs, medication administration, vital signs monitoring, and behavior management will prepare you for success in this role. Familiarity with EMR systems, group home environments, home & community care experience, and use of mobility aids like Hoyer lifts is preferred. Certification in first aid is desirable. Candidates should demonstrate excellent communication skills, patience, empathy, and the ability to work effectively within a team dedicated to providing high-quality senior care. A valid driver’s license is a plus for those who will be driving residents safely within the community.
