As a Community-Based Service Provider (CBSP), you will deliver direct services to children and their caregivers enrolled in Children & Family Treatment Supports and Services (CFTSS) and Home & Community Based Services (HCBS) programs. Using a person-centered planning approach, you will engage youth and caregivers to develop personalized treatment plans and identify future goals.
Responsibilities:
- The FT CBSP is expected to provide 95% of their assigned client's weekly services.
- The FT CBSP is expected to complete 96 hours of service provision per month.
- The FT CBSP will maintain necessary technology (phone, internet, video camera) to perform job requirements, functions of documenting, coordinating, service planning and billing.
- Successful candidate will be responsible for all service related tasks including the completion and submission of notes within 48-72 hours of service rendition therefore, the CBSP must have consistent access to a computer/laptop, internet and flexibility to attend meetings and communicate with Managed Care Organizations.
- Attend all mandated trainings including, but not limited to: Mandated Reporter, Evolv, Service-based trainings, Clinical Supervision, and other skill developing, best-practices trainings.
- Maintain a service calendar, for each assigned youth, documenting changes or missed services.
- Participate in interdisciplinary team meetings held for client caseload.
Requirements:
- Bachelor's Degree in Human Services or related field.
- Two years of experience working with children to support their development and wellbeing.
- Knowledge of mental health diagnoses, symptomatic behaviors, and trauma responses.
- Flexible schedule (including evenings and weekends).
- Bilingual English-Spanish required.
- Strong communication (oral and written) and technology skills.
- Effective engagement skills; creative and resourceful.
Position requires to travel to Westchester County to provide services
