Primary Work Location:  

MULTISITE: Multisite staff work across several program locations throughout Alameda County, providing care and support wherever it is most needed. These sites offer a range of services, including post-hospitalization medically supported shelter, medical respite care, skilled nursing, and caregiving support. 


Job Summary:

Cardea Health is a nonprofit healthcare organization dedicated to providing compassionate clinical services to vulnerable populations. We currently have an opening for a Physical Therapist. Be an integral part of a team focused on the rehabilitation and recovery of patients. This may include providing care to individuals in unstable and/or non-traditional housing environments, such as shelters or unsheltered settings. The Physical Therapist will be responsible for both the evaluation and treatment of their patient caseload. 

Qualifications:

Responsibilities: 

 

  • Perform initial and ongoing clinical assessments to identify patient needs, determine level of functioning 

  • Provide therapeutic treatments and evaluation of equipment needs as applicable 

  • Evaluate outcomes of care with appropriate discharge planning 

  • Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease 

  • Accept clinical assignments that are consistent with education and competence to care for patients 

  • Complete appropriate documentation in a timely manner 

  • Meet mandatory continuing education requirements of the agency/licensing board and participate in in-service programs 

  • Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate 

  • Promote company philosophy and administrative policies 

  • Demonstrate commitment, professional growth, and competency 

  • Other duties as assigned 

 

Minimum Required Skills & Qualifications:  

 

  • Education: Bachelor of Science in Physical Therapy (or Doctor of Physical Therapy) from a CAPTE-accredited program 
  • Licensure: Current California State License as a Physical Therapist and current California Driver’s License 
  • Skills & Certifications:
    • Therapy skills in alignment with generally accepted standards of physical therapy practice 
    • Strong interpersonal and communication skills 

    • Proof of current CPR certification 

    • Knowledge of durable medical equipment

 

Preferred Skills & Qualifications:  

 

  • 1 years of Physical therapy experience (preferred)

 

Physical Requirements:  

 

  • Occasional sitting or stationary work 

  • Frequent standing, walking, kneeling, stooping, and ascending stairs 

  • Occasional lifting and carrying up to 20 lbs 

  • Frequent pushing and pulling 

  • Frequent reaching high, low, and level 

 

Work Environment:  

 

  • In-person onsite at a supportive housing facility. Constant work around and/or with people OR Interaction with patients, clients, and external customers
