What You’ll Do
As an LPN on our team, you will:
Provide compassionate, direct nursing care to residents while following individualized care plans
Administer medications and treatments accurately and safely
Closely monitor residents’ conditions, promptly identify changes, and communicate effectively with RNs and physicians
Maintain thorough, timely, and accurate documentation
Guide and supervise Certified Nursing Assistants (CNAs) to ensure consistent, high-quality care
Support smooth admissions, transfers, and discharges
Build trusting relationships with residents, families, and the entire care team through clear and caring communication
Uphold the highest standards of infection control and resident safety
What You Bring
Active and unrestricted LPN license in the state of Pennsylvania
Passion for geriatric and rehabilitative care
Strong clinical skills, attention to detail, and a collaborative spirit
