Job Title: Housing Case Manager (Full-Time)

Job Summary:

The Housing Case Manager will deliver services to individuals and families experiencing or at risk of homelessness.

The Case Manager assists clients with housing search. Assesses housing needs, identifies housing resources, facilitates housing applications, and works collaboratively with landlords and property managers to place individuals and families in housing. Position involves travel around the county, client transportation, and in-home services. The position requires some evening work and flexibility for client needs and involves traveling to various locations including client homes in the Saratoga County service area.

Duties and Responsibilities:

· Delivering housing related services to the homeless population and those at risk of homelessness throughout Saratoga County; to include assessment of housing needs, identification of housing resources, completing applications, facilitating rental assistance and utility assistance, moving assistance, obtainment of furniture and household necessities, food and clothing.

· Process financial payments to landlords per agency approved procedures

· Collaboration with youth street outreach program to complete street shifts to locate clients and to serve the families of runaway youth with housing needs.

· Work closely with landlords and property managers. Visit mobile home parks regularly to provide eviction prevention services to families with children, establishing relationships with park management offices.

· Provide ongoing case management to include maintenance of case files and coordination of services with appropriate agencies for additional services.

• Meet and provide outreach to community members, landlords, businesses, local service organizations, school district officials and other concerned neighbors within Saratoga County to promote CAPTAIN CHS and the program as needed.
• Attend monthly Coordinated Entry meetings for Saratoga Counties and enter eligible clients into the Coordinated Entry List as needed.
• Complete HMIS case data entries per requirements.
• Ensure all quarterly and annual reports are completed and submitted on time.
• Complete and document 10 hours of job-related training per year.
• Perform other related work as assigned.

Skills Required:

• Ability to work with all ages and income levels and be a dedicated client advocate.
• Knowledge of community resources.
• Good Communication, networking and public relations skills.
• Able to work flexible and irregular hours as needed.
• Able to multi-task and work in a team environment.
• Solid working knowledge of Microsoft Office products, social media sites and some web-based applications.
• Familiarity with Bridges Out of Poverty constructs (training provided)

Qualifications:

• Bachelor’s (preferred) or associate’s degree from an accredited college or university in human services, counseling, education or related field.
• Minimum of 2 years’ experience in the field of human services, homelessness services preferred.
• Must possess a valid NYS Drivers License, clear a DMV check through the Agency insurer, and provide evidence of appropriate insurance coverage.
• Ability to lift 30lbs.

Telecommute and Remote Work:

The primary functions of this position require in-office and significant in-community services. Occasionally, a project assignment could be accomplished from home.
