Responsibilities
- Provide strategic leadership and administrative oversight for all home care nursing operations, ensuring alignment with organizational goals and regulatory standards.
- Supervise and mentor nursing management teams, fostering professional development and maintaining high-performance standards across all clinical staff.
- Develop, implement, and monitor comprehensive care plans tailored to individual patient needs, including residents in assisted living, memory care, hospice, and long-term care settings.
- Ensure compliance with federal and state regulations such as Medicare requirements, long-term care regulations, HIPAA privacy rules, and other applicable guidelines.
- Oversee documentation review processes to maintain accuracy in medical records, medical terminology entries, MDS (Minimum Data Set) assessments, and clinical documentation within EMR/EHR systems.
- Manage staffing schedules across shifts to ensure optimal patient coverage while maintaining staff well-being and regulatory compliance.
- Lead quality improvement initiatives focused on patient safety, satisfaction, and continuous clinical excellence in areas such as memory care, acute care episodes, and chronic disease management.
- Stay current on industry trends related to home health agency operations, Medicare policies, and long-term care regulations to inform best practices.
Qualifications
- Proven nursing leadership experience with a strong background in home health services or long-term care facilities.
- Extensive knowledge of resident’s rights policies, medical terminology, anatomy and physiology principles relevant to elderly or chronically ill populations.
- Demonstrated success managing multidisciplinary teams within assisted living environments or nursing homes with supervising experience in clinical settings.
- Familiarity with EMR/EHR systems used for documentation review, medical records management, and regulatory reporting.
- Strong understanding of Medicare regulations, Medicaid/Medicare billing processes, and compliance standards governing home health agencies.
- Excellent communication skills with the ability to lead change effectively while fostering a positive team culture.
- Ability to navigate complex regulatory environments related to long-term care facilities while ensuring adherence to HIPAA privacy rules.
