Role Description This full-time, on-site Registered Nurse Case Manager role is based in Amarillo, TX and focuses on providing coordinated, patient-centered home health care. The Registered Nurse Case Manager assesses client needs, develops individualized care plans, and oversees the implementation of nursing services in the home setting. Daily responsibilities include conducting patient visits, monitoring clinical status, administering treatments, and educating clients and caregivers on health management and safety. The role also involves collaborating with physicians and interdisciplinary team members, updating documentation and care plans, and ensuring compliance with regulatory and agency standards. The Registered Nurse Case Manager plays a key role in promoting positive health outcomes and supporting clients and their families through ongoing communication and advocacy.
Qualifications
- Current Registered Nurse (RN) license in the state of Texas and eligibility to practice without restrictions.
- Demonstrated clinical assessment, care planning, and case management skills in home health or related settings.
- Strong patient education, communication, and interpersonal abilities to work effectively with clients, families, and multidisciplinary teams.
- Solid organizational and time-management skills, including the ability to prioritize visits, manage caseloads, and complete documentation accurately and promptly.
- Knowledge of home health regulations, standards of care, and best practices in infection control and patient safety.
- Comfort with electronic health record systems and basic computer skills for documentation and communication.
- Ability to travel locally for in-home visits and perform the physical tasks associated with direct patient care.
- Prior home health, case management, or community nursing experience preferred.
- Associate or Bachelor’s degree in Nursing from an accredited program; BSN preferred.
