Role Description
This is a full-time on-site role for a Registered Nurse (RN) Case Manager located in Abilene, TX. The RN Case Manager will oversee patient care plans to ensure quality and continuity of care for clients. Responsibilities include assessing patient needs, planning and coordinating care, monitoring patient progress, educating patients and families, and collaborating with interdisciplinary teams. Additional duties may involve maintaining accurate medical records and ensuring compliance with healthcare regulations and organizational policies.
Qualifications
- Strong clinical skills and expertise in patient assessment and care planning
- Ability to coordinate care and collaborate effectively with interdisciplinary medical teams
- Proficiency in maintaining accurate and compliant medical documentation
- Excellent communication and patient education skills
- Valid Registered Nurse (RN) license in the state of Texas
- Experience in case management and home health services is preferred
- Strong organizational and time management skills
- CPR certification and a valid driver’s license with reliable transportation
