Role Description The Remote Placement Coordinator is a full-time, on-site role based in McKinney, TX, responsible for managing the end-to-end placement process for individuals transitioning from hospitals or other care settings into residential environments. On a day-to-day basis, this role gathers and reviews referral information, communicates with families, case managers, and healthcare professionals, and identifies appropriate placement options within the verified provider network. The coordinator conducts needs assessments, discusses care preferences and budgets, presents suitable care options, and supports informed decision-making. Responsibilities include maintaining accurate records, updating case status, scheduling tours or introductions with providers, and ensuring smooth coordination of move-in logistics and follow-up. The role also involves responding to time-sensitive referrals, collaborating closely with internal care planning staff (including RN support), and continuously monitoring placement quality and client satisfaction.
Qualifications
