Description

We are looking for a full-time LPN, this position is benefit eligible.

We have the following positions available:

  • Sunday through Thursday from 2pm to 10:30pm

Provide positive and supportive leadership and direction of the shift nursing services of Manor Care in accordance with current federal, state and local standards, guidelines and regulations governing our facility and as may be directed by the Executive Director/Administrator, or the Director of Nursing Services (DNS) to ensure that the highest degree of quality care is maintained at all times. Support and assist Supervisory Medication Aide (SCMA) on 2nd unit as necessary, including as specifically indicated after "A" in list of duties and responsibilities.

PRINCIPLE DUTIES:

Essential Job Duties:

  • Support the mission, homelike environment, philosophy, policies, standards and procedures of Capital Manor. Adheres to and supervises staff under the established policies and procedures, including fire and safety policies and precautions.
  • Work at all times within and under the scope of standards defined and required by any licensure or certification for this position, as well as Capital Manor specific training, policies and procedures for this position as team leader
  • Assume the administrative authority, responsibility and accountability necessary for successfully performing the duties of Licensed Nurse.
  • Make a minimum of twice a shift resident observation and evaluate the residents' medical status and needs.  Complete “Alert Status” observations and documentation.
    • Assess/observe all residents in unit having acute medication needs or changes of condition.  Document according to observations following “alert charting” policy.
  • Assist in coordinating nursing services with residents’ PCP, Rehab, Infusion, Hospice, etc. services to ensure the continuity of the residents’ total regimen of care.
  • Complete admission reviews and observations on residents admitting to Manor Care and all residents with changes and/or alert status. 
    • Respond to falls and coordinate care with staff.
    • Review faxes have been sent and responded to or received. If response has not been received within 3 days, the concern needs to be re-faxed or called.
    • Help facilitate processing of all new orders, assuring completion and signing off for accuracy.
  • Complete documentation including nursing assessments (RN only), observations, and all nursing clinical records according to policy and procedures.
    • Document all observations and/or assessments and care provided as needed during your shift.
    • Complete occurrence/incident record form with all incidents including thorough investigation and identification of why incident occurred.
    • Assist in timely completion of pain observations, bed assistive device evaluation (RN only), and any other resident evaluations/observations.  Review pharmacy suggestions and incoming laboratory reports.  Notify physician of abnormal results needing further orders, and signing and dating completion.
    • Read and provide timely information in the communication book, 24-hour report and all service plans.
  • RN only-Teach, support, and observe staff in following the established nursing and facility policies and procedures.
    • Teach direct care staff preventative measures to maintain residents at their highest functional levels.
  • Administer all injectable medications and treatments beyond CMA scope of practice unless area where delegation has been completed.
    • Administer all insulin to residents according to facility protocol and PCP order.
  • Receive and process incoming medical providers’ orders timely.  Sign and date with completion. 
  • Other duties as assigned

Other Responsibilities and Requirements:

  • Develop additional health information records policies and procedures as needed under direction of consultant and nursing staff.
  • Order medical equipment as necessary by forwarding supply needs to Lead Nurse for ordering.
  • Coordinate or respond to independent resident emergencies when Support Services staff is unavailable.
  • Recommend to the Lead Nurse or DON any equipment or supply needs of the department.
  • Possess knowledge of nursing and medical practices and procedures as well as the laws, regulations, guidelines, and scope of practice.
  • Inspect the nursing service treatment and medication areas daily to ensure that they are maintained in a clean and safe manner.
  • Attend and participate in continuing education programs, in-service meetings and other meetings as required/directed.
  • Serve on, participate in, and attend various committees of the facility.
  • Work beyond normal working hours, on weekends and holidays, and in other positions/shifts when required/directed.
  • Work harmoniously with all personnel, professional and non-professional, in all departments of CM.
  • Keep communication lines open with residents, family or responsible parties and team members.
  • Work independently, displaying flexibility, personal integrity, patience, enthusiasm, professionalism and a cheerful disposition.
  • Deal tactfully with residents, family members, personnel, and all visitors at CM.
  • Preserve the residents’ dignity, rights, and self-respect at all times.
  • Assure that residents’ personal and property rights are observed at all times.
  • Maintain confidentiality of all resident information.
  • Assist in creating and maintaining an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout Capital Manor.
  • Demonstrates support of continual evaluations, restructuring, and trying alternative approaches for delivery of services.

Qualifications


QUALIFICATIONS:

  • Possess, at minimum, a Nursing Diploma from an accredited college, university or nursing school, and a current, unencumbered license to practice as a Registered Nurse or Licensed Practical Nurse in Oregon.
  • One-year experience desired, working in a hospital, long-term care facility, or other related health care facility.
  • Training in rehabilitative and restorative nursing practices.
  • Possess knowledge of computer systems, system applications, and other office equipment.
  • Maintain current CPR/First Aid/Food Handlers cards.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel; and talk or hear.  The employee frequently is required to stand and walk throughout the shift and on all units. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds. Must have visual and hearing acuity. Must be able to appropriately respond physically and mentally to situations. Must be able to assist in the evacuation of residents.
