$75,000$95,000 per year

What Youll Do
Perform pre-admission evaluations to determine hospice eligibility
Educate patients and families about hospice services and the admission process
Obtain informed consent and initiate the plan of care
Complete nursing assessments and develop an initial care plan focused on comfort and quality of life
Provide compassionate support to patients and families during the transition to hospice
