Overview
We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to provide exceptional patient care for those in need covering all of Cape Cod. The ideal candidates will have experience working with diverse patient populations, including seniors, individuals with developmental disabilities, dementia, and hospice needs. This role offers an opportunity to make a meaningful difference in patients' lives through personalized care and support. Responsibilities
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support.
- Monitor and observe patients' conditions, reporting any changes or concerns promptly.
- Administer medication and perform basic medical tasks such as taking vital signs.
- Run errands, doctors appointments etc.
- Support patients with meal preparation and feeding as needed
- Assist with light housekeeping duties including laundry and cleaning to maintain a safe environment.
- Provide compassionate care tailored to individual needs in memory care, dementia care, hospice care, and long-term care settings.
- Work collaboratively with healthcare teams to ensure comprehensive patient care plans are followed.
- Maintain medical records diligently and ensure compliance with all healthcare standards.
Qualifications
- Valid CNA certification or HHA certification required; prior experience in nursing homes or assisted living preferred
- Experience working with people with developmental disabilities or dementia is highly desirable
- Valid Drivers License
- CPR certification required
- Strong interpersonal skills with the ability to work compassionately with diverse patient populations.
- First Aid Certified.
- Experience with medication administration and vital signs measurement preferred.
- Familiarity with HIPAA regulations and maintaining patient confidentiality
- Prior experience in home & community care or long-term care settings is advantageous.
