CLS/COMPANION JOB DESCRIPTION
Description: Provides client support services, under the supervision of the appropriate professional staff. Companion shall assist the client and the client’s family in the achievement of physical goals and emotional comfort. Provide Training and assistance per (POC) Plan of Care. The long-term goal is the client achieves Plan of Care Objectives independently.
Job Duties/Knowledge
Provides Personal care services per state/federal rules, regulations, and Capacity Care policies and procedures:
Medication:
o Medication Reminders Only
o Medication Administration for ABI clients only if trained and in POC.
o Assist in maintenance of prescriptions, refill requests and documentation.
o Participates in ongoing training related to medication.
Training or Assistance with ADL, which may include (Not all clients require):
o Bed, sponge, tub, or shower bathing assistance in safe manner.
o Shampooing hair in sink, tub, or shower
o Caring for nails and skin (Do Not cut nails)
o Oral hygiene
o Assisting with toileting and elimination, changing depends if necessary.
o Ensures adequate nutrition and fluid is being taken by the client.
o Assisting with clothing maintenance and dressing.
Light Housekeeping training or assistance: 1
o Changing bed linens
o Light housekeeping
o Laundry with assistance to clients if appropriate.
o Meal Preparation assistance.
Safety and Health:
o Maintaining a clean, safe, and healthy environment
o Apply safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care. Such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning.
o Observes, reports and documents change in client status. (Must possess critical thinking skills)
o Understands basic elements of body functioning and reports changes in client’s body functions as indicated.
o Recognizes emergency situations and implements appropriate emergency procedures per agency policy. (Ability to carry out directions)
o Follows agency procedures about infection control, handling of hazardous wastes, and safety measures.
Documentation and General:
o Accurately documents care provided and submits notes and other required documents per agency policy. (Must possess the ability to read and write)
o Understands and utilizes (EVV) Electronic Visit Verification system.
o Attends agency provided in-service programs to fulfill requirements of position and agency policies. Must complete 6 hours of continuing education each year. (Ability to deal with demands of the job)
o Follows specific written instructions for personal care in accordance with the Client’s Plan of Care.
o Has an empathic, pleasant, and caring attitude toward clients, client family and fellow employees.
o Has a team- orientated attitude toward peers.
o Has a continual desire to learn and become more proficient in position
o I am willing to listen and examine other opinions.
Qualifications
o Meets federal/state requirements for this position.
o Able to work independently with minimal direct supervision.
o Passes tests and competence related to agency training.
o Able to work as a team member.
o Current CPR certification provided to the agency.
o Current TB test provided to agency.
o Minimum of High school degree or equivalent
o Ability to pass random drug screens.
Ability to pass random criminal background checks.
Degree of Travel
Transportation to and from work assignments is not a reimbursed expense.
Evidence of current driver’s license and liability insurance is required.
In instances where employees perform tasks that require the use of his/her automobile,
A mileage report must be turned in for reimbursement.

Base Pay: From $15.00 per hour
