The CNA regularly travels to patient homes and utilizes electronic health records to document care effectively.
Responsibilities
- Provide direct patient care and support personal hygiene needs
- Communicate effectively with patients, families, and healthcare team members
- Assist in monitoring patient conditions and reporting changes
- Ensure accurate documentation using electronic health record systems
- Collaborate with nursing and hospice staff to coordinate patient care
- Travel regularly to patient homes to deliver care
Preferred Qualifications
- Entry-level experience in home health or hospice care
- Certified Nursing Assistant (CNA) license
- Patient care skills
- Basic Life Support (BLS) certification
- Strong time management abilities
- Proficient in documentation and electronic health record systems
- Effective team collaboration skills
