What are the responsibilities and job description for the Intake Specialist position at Canyon Home Care & Hospice LLC?
We are looking for a detail-oriented Intake Coordinator to support our field clinical team through accurate insurance verification, benefits investigation, and chart preparation. This is a back-office administrative role — you will work closely with internal staff and payers, ensuring field clinicians have everything they need to deliver care before they ever step through a patient’s door.
Verify insurance eligibility and active coverage for new and existing patients using payer portals and phone inquiries
Investigate and document detailed benefits information including deductibles, co-pays, out-of-pocket maximums, and visit limitations
Obtain prior authorizations and referrals required for services delivered by field clinicians
Build and organize patient charts, ensuring all demographic, insurance, and clinical intake documentation is complete and accurate prior to the first visit
Track authorization statuses and proactively request renewals to prevent lapses in coverage
Communicate benefits findings and any coverage limitations clearly to the clinical and billing teams
Identify and resolve intake discrepancies or missing documentation before clinician deployment
Maintain organized, audit-ready records in compliance with HIPAA and organizational standards
High school diploma or GED required; associate’s degree in healthcare administration preferred
2 years of experience in insurance verification, benefits investigation, or medical intake in a healthcare setting preferred.
Solid understanding of commercial, Medicare, and Medicaid benefit structures and authorization requirements
Experience working in Epic, CarePort and similar portals
High accuracy and strong organizational skills with the ability to manage multiple charts simultaneously
Ability to work independently, prioritize workload, and meet deadlines in a fast-paced environment
Strong written communication skills for internal documentation and cross-team correspondence
Knowledge of HIPAA regulations and commitment to patient data privacy
