Job Responsibilities
- Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT)
- Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT
- Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions
- Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities
- Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments
- Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies
- Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist
- Complete accurate and timely electronic documentation in compliance with Medicare home health regulations
- Follow infection control standards and home health safety protocols
- Participate in QAPI and agency performance improvement initiatives

Qualifications
- Associate degree from an accredited Physical Therapist Assistant (PTA) program
- Active Physical Therapist Assistant (PTA) license in the state of Texas
- Home Health experience (1 year required)
- HomeCareHomeBase (HCHB) experience (preferred)
- Current CPR certification
- Strong communication, clinical judgment, and time-management skills
- Valid driver’s license, reliable transportation, and current auto insurance
