Position Summary
The Occupational Therapist (OT) provides patient-centered occupational therapy services in Skilled Nursing, Long-Term Care, and Home Health settings. Key Responsibilities
- Evaluate patients and develop individualized occupational therapy plans of care
- Provide skilled occupational therapy treatments focused on ADLs, IADLs, cognition, strength, coordination, and functional mobility
- Supervise and collaborate with Occupational Therapy Assistants (OTA/COTA) in accordance with state practice acts
- Monitor patient progress, modify plans of care as appropriate, and communicate changes to the interdisciplinary team
- Educate patients and caregivers on therapeutic techniques, safety strategies, adaptive equipment, and home modifications
- Complete timely, accurate, and compliant documentation in the electronic medical record (EMR)
- Ensure compliance with all federal, state, payer, and facility regulations, including HIPAA
- Maintain a safe treatment environment and adhere to infection control and safety protocols
- Demonstrate professionalism, compassion, and respect in all patient and family interactions
Qualifications
- Graduate of an accredited Occupational Therapy program
- Current Occupational Therapist license in good standing (state-specific)
- Experience in skilled nursing, long-term care, or home health preferred (new graduates welcome to apply)
- Strong clinical, communication, and time-management skills
- Ability to work independently while contributing to a collaborative team environment
