About ProCare Advantage & Cantex Continuing Care Network
ProCare Advantage is a specialized health plan under the Cantex Continuing Care Network, a leading provider of post-acute care services. Our mission is to deliver innovative, patient-centered solutions that improve health outcomes and quality of life. Through our ESRD and Diabetes Chronic Special Needs Plans (CSNP), we focus on supporting individuals with complex health needs by offering personalized care and strong community partnerships.
We are seeking a Community Engagement Manager to lead outreach and engagement strategies that drive growth and brand awareness for ProCare Advantage’s ESRD and Diabetes CSNP offerings. This individual will serve as the face of this new initiative, building strong relationships with community stakeholders, healthcare providers, and local organizations to support enrollment and retention.
Qualifications
Key Responsibilities
- Develop and execute community engagement strategies to increase awareness and enrollment in ESRD and Diabetes CSNP plans.
- Identify and engage with physicians, dialysis centers, and other healthcare settings to establish partnerships and provider relationships for CSNP members.
- Build and maintain relationships with local organizations, healthcare providers, and community leaders to support plan growth.
- Organize and manage community events, outreach programs, and educational initiatives focused on chronic condition management.
- Collaborate with internal teams (marketing, clinical, operations) to align engagement efforts with organizational goals.
- Monitor and report on engagement and growth metrics, providing actionable insights for continuous improvement.
- Represent ProCare Advantage at virtual and in-person community meetings, forums, and public events within DFW.
Qualifications
Required:
- 3 years of experience in community engagement, outreach, or healthcare marketing.
- Strong understanding of Medicare Advantage or Physician office setting.
- Excellent communication and interpersonal skills with the ability to build trust and rapport.
- Must reside in the Dallas–Fort Worth metroplex and be available for occasional local travel.
Preferred:
- Experience in healthcare or population health initiatives.
- Knowledge of ESRD and Diabetes care programs and community resources.
- Bilingual proficiency (Spanish/English) is a plus.
- Bachelor's Degree
- Clinical license- LVN or RN
- Familiarity with CRM tools and social media platforms for engagement.
