We are seeking an experienced and detail-oriented MDS Coordinator. This role is responsible for conducting resident assessments and completing the Minimum Data Set (MDS) in compliance with federal, state, and local regulations. The MDS Coordinator plays a key role in care planning, reimbursement accuracy, and ensuring the highest quality of resident care. This position is a full-time, salaried position, Monday through Friday, with inclusion in the weekend on-call nursing rotation. Salary will be based upon experience.Key ResponsibilitiesComplete MDS assessments accurately and in a timely manner according to established schedules and regulations.Assess residents routinely and as needed based on condition changes, readmissions, or care needs.Communicate resident condition changes to appropriate clinical staff.Coordinate with interdisciplinary team members including Nursing, Social Services, Activities, and Dietary.Oversee care plans to ensure accuracy and compliance with resident needs and regulatory standards.Prepare and distribute MDS schedules to interdisciplinary teams.Maintain accurate documentation and transmit MDS data to state systems as required.Generate and review Quality Indicator and clinical reports.Participate in care planning meetings and reimbursement level determinations.Promote resident dignity, rights, and safety at all times.Follow infection control, safety, and confidentiality policies.Report any incidents, exposures, or compliance concerns per facility protocol.QualificationsDegree in Nursing from an accredited school required.Active, unencumbered RN or LPN license in Connecticut.Previous long-term care experience strongly preferred.Knowledge of aging, chronic illness, and long-term care regulations.Experience with MDS assessments and care planning preferred.Skills & CompetenciesStrong clinical judgment and assessment skillsExcellent written and verbal communicationWhy Join UsSupportive leadership and collaborative care environmentOpportunity to make a meaningful impact on resident outcomesCompetitive compensation and benefitsStable, professional work setting in long-term carePlease reply to vbrogden@theatlashcg.com
