Responsibilities
- Dispense prescription medications accurately, adhering to all legal and safety standards, verify non-sterile compounded prescriptions.
- Conduct thorough patient assessments to determine appropriate medication therapy, considering physiology, anatomy, and medical history.
- Provide expert patient service by offering medication counseling, immunizations, and education on medication administration and potential side effects.
- Manage inventory efficiently, including stock replenishment, medication storage compliance, and proper documentation within pharmacy software systems.
- Collaborate with healthcare teams on medical management plans, especially in acute care, pain management, pediatrics, and LTC (Long-Term Care) pharmacy settings.
- Maintain compliance with HIPAA regulations and ensure all patient information is handled confidentially within EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems.
- Support pharmacy technician staff by overseeing medication preparation processes and ensuring adherence to safety protocols.
Qualifications
- Valid pharmacist license with current registration in the applicable jurisdiction.
- Proven experience in clinical pharmacy practice, including hospital or acute care settings; LTC pharmacy experience is a plus.
- Strong knowledge of medical terminology, physiology, anatomy, and pharmacology principles related to patient care.
- Experience with pharmacy software systems for prescription processing and inventory management.
- Familiarity with compounding medications in a non-sterile environment.
- Demonstrated ability to perform patient assessments, medication reviews, and provide immunizations confidently.
- Prior pharmacy residency or specialized training in critical care or hospital pharmacy is highly desirable.
- Excellent communication skills with a focus on patient service and interdisciplinary collaboration.
