Campo Behavioral Health, LLC is seeking a compassionate, dependable, LPN to support individuals with intellectual and developmental disabilities through New Mexico’s Developmental Disabilities Waiver services.

This position is ideal for a nurse who values person-centered care and wants to make a meaningful difference in the lives of individuals receiving Supported Living and Customized Community Supports services.

Duties

  • Report to work on time and prepared to fulfill assigned duties and scheduled hours.
  • Provide nursing services under the direction and supervision of the Registered Nurse (RN).
  • Complete all required documentation accurately, timely, and in accordance with DDSD, Medicaid Waiver, agency, and regulatory requirements.
  • Review medical contact documentation and ensure all required RN reviews and co-signatures are obtained.
  • Coordinate and monitor medical, dental, therapy, and specialty appointments to ensure continuity of care.
  • Monitor the health status of individuals receiving services and promptly report changes in condition to the RN, physician, or other appropriate healthcare provider.
  • Complete health assessments, nursing observations, and the Consumer Problem Checklist as directed by the RN.
  • Administer medications and treatments within the scope of LPN practice and in accordance with physician orders, agency policy, and state regulations.
  • Ensure physician orders are current, accurately transcribed, and implemented.
  • Train, monitor, and document competency of Direct Support Professionals (DSPs) on delegated nursing tasks and physician-ordered procedures under the direction of the RN.
  • Participate in nursing quality assurance activities, including medication administration record (MAR) reviews, medication storage audits, health record audits, infection control monitoring, and other quality improvement activities.
  • Maintain complete, accurate, and organized medical records in both the residential setting and the electronic health record (THERAP).
  • Upload required nursing documentation, vital signs, physician orders, and other health-related documents into THERAP in a timely manner.
  • Assist in developing, implementing, and monitoring Individual Support Plans (ISPs) and healthcare plans as directed by the RN.
  • Maintain professional communication with individuals served, guardians, families, physicians, pharmacies, hospitals, therapists, case managers, interdisciplinary team members, and agency staff.
  • Maintain confidentiality of all protected health information in accordance with HIPAA, DDSD requirements, and agency policy.
  • Immediately report suspected abuse, neglect, exploitation, deaths, environmental hazards, medication errors, or other reportable incidents in accordance with the New Mexico DDSD Incident Management System and agency policy.
  • Attend required staff meetings, nursing meetings, trainings, competency reviews, and continuing education.
  • Maintain an active, unrestricted New Mexico LPN license and all required certifications, including CPR/First Aid.
  • Maintain reliable transportation, a valid driver's license, current automobile insurance, and an acceptable driving record.
  • Participate in the agency's 24-hour nursing on-call rotation as assigned by the RN.
  • Maintain an assigned caseload as determined by the RN Supervisor based on agency needs.
  • Perform other duties as assigned by the RN Supervisor or Director of Nursing that are consistent with the position and within the scope of practice.

Minimum Qualifications

  • Current New Mexico LPN license in good standing.
  • Strong clinical assessment, documentation, communication, and organizational skills.
  • Ability to work independently
  • Ability to complete required background checks and employment screenings.
  • Experience with medication administration, staff training, chronic health conditions, or community-based nursing is preferred.

Experience working with individuals with intellectual or developmental disabilities is strongly preferred, but qualified nurses who are willing to learn the Developmental Disabilities Waiver system are encouraged to apply.
