The hours for this position are Monday through Friday 10:30 am to 7pm
Responsibilities:
The RN Registered Nurse Admissions and Discharges is primarily responsible for the following job duties:
Assists with ensuring rooms are ready for new admissions- Greet new patients warmly and orient them to their new environment
- Complete admission assessments and documentation, including initiation of the baseline care plans
- Communicate patient status with interdisciplinary team
- Complete medication reconciliation and order entry
- Obtain consents if needed
- Assist with patient and caregiver education
- Ensure discharge orders are accurate
- Complete discharge documentation
- Complete comprehensive discharge education on post-discharge instructions, including wound care, or new medications.
- Coordinate post discharge transition of care appointments
- Complete post discharge follow up calls and documentation
- Help identify patients at high risk for rehospitalization and implement early interventions
- Work collaboratively with interdisciplinary team members to ensure smooth transition
- Complete admission paperwork with the resident/resident representative (MA 401)
- Assist with other duties as assigned
- Must be a graduate of an accredited school of nursing, college, or university; BSN preferred.
- Current Registered Nurse licensure by the State Board of Nursing
