Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and document patient health status by observing vital signs, recognizing changes in condition, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Administer medications following prescribed protocols and ensure proper medication documentation in compliance with HIPAA and home health regulations.
- Support patients with medical procedures such as catheter care, wound dressing changes, and basic first aid or CPR when necessary.
- Provide companionship and emotional support to clients with dementia or Alzheimer’s disease, ensuring a safe environment for memory care and geriatric patients.
- Assist with meal preparation, light cleaning, laundry, and maintaining a clean and safe living environment for clients.
- Communicate effectively with clients’ families and healthcare teams to coordinate care plans and update medical records accurately.
Qualifications
- Proven experience working as a Home Health Aide or Caregiver in assisted living facilities, nursing homes, hospice care, or home healthcare settings.
- Knowledge of medical terminology, patient observation techniques, and home health care assistance standards.
- Experience working with individuals with developmental disabilities, dementia, Alzheimer’s disease, or other cognitive impairments.
- Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts.
- Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications in hospice or end-of-life care are a plus.
- Familiarity with EMR/EHR systems for medical documentation; understanding of home health agency regulations is preferred.
- Strong communication skills for client interaction and family updates while maintaining patient confidentiality per HIPAA guidelines.
- Ability to work independently in live-in employment settings or flexible schedules that meet client needs.

Pay: $16.00 - $17.00 per hour
