Essential Responsibilities
Complete comprehensive nutrition assessments for new admissions and as required by regulations.
Develop, implement, and monitor individualized nutrition care plans.
Conduct resident interviews to obtain food preferences and complete Resident Food Profiles.
Update resident food preferences in the dietary management/tray card system.
Monitor residents for changes in nutritional status, weight, skin integrity, hydration, and swallowing concerns.
Collaborate with physicians, nursing, speech therapy, and other interdisciplinary team members to optimize resident outcomes.
Recommend therapeutic diets and nutrition interventions based on physician orders and clinical needs.
Provide nutrition education to residents, families, and staff.
Participate in care plan meetings and quality assurance/performance improvement initiatives.
Ensure compliance with CMS, state, and federal regulations related to dietary services.
Review and monitor meal service for quality, accuracy, and resident satisfaction.
Assist with regulatory surveys and implement corrective actions when necessary.
Refer residents with specialized nutritional concerns for additional evaluation as appropriate.
Maintain accurate and timely documentation in the electronic medical record.
Qualifications
Required
Registered Dietitian (RD/RDN) credential with the Commission on Dietetic Registration (CDR)
Licensed Dietitian in the State of Missouri (or eligible for licensure)
Bachelor's degree in Dietetics, Nutrition, or related field
Knowledge of clinical nutrition, therapeutic diets, and medical nutrition therapy
Strong organizational, communication, and time management skills
Basic computer proficiency and experience with electronic medical records
Ability to work independently while collaborating effectively with an interdisciplinary team
Preferred
Experience in skilled nursing, long-term care, rehabilitation, or geriatric nutrition
Experience with PointClickCare or other electronic health records
Knowledge of CMS regulations and survey process
