The MDS Coordinator (RN) is responsible for the accurate and timely completion of the RAI/MDS process from admission through discharge in compliance with federal, state, and regulatory guidelines. This role works closely with the Director of Nursing and interdisciplinary team to ensure care plans and documentation accurately reflect resident needs and services provided.
Key Responsibilities
- Coordinate and complete MDS assessments in accordance with RAI guidelines
- Collaborate with the Interdisciplinary Team to develop and maintain resident care plans
- Monitor documentation to ensure accuracy and regulatory compliance
- Manage MDS scheduling, tracking, and submission to state databases
- Ensure timely follow-up on validation errors and maintain submission records
- Participate in daily stand-up meetings, care plan meetings, and QA processes
- Identify significant changes in resident condition and ensure timely updates
- Assist with survey preparation and correction of deficiencies
- Utilize PointClickCare for MDS, discharge/re-entry, and Medicaid tracking
- Report audit trends and findings to Quality Assurance Committee
- Current RN license in the state of Missouri (required)
- Minimum 2 years of skilled nursing facility experience preferred
- Strong knowledge of:
- MDS 3.0
- Medicare PPS & Medicaid reimbursement
- OBRA regulations & Quality Indicators
- Experience with PointClickCare preferred
- Detail-oriented with strong organizational and communication skills
