POSITION SUMMARY:

The In-Community Clinical & Recovery Engagement Specialist provides outreach, engagement, counseling, peer support, care coordination, and harm-reduction-aligned services in community settings to individuals with Substance Use Disorders (SUD) and Co-Occurring Disorders.

This position supports Camelot’s mobile, low-threshold service model by delivering the same high-quality care offered in the clinic directly in the field, including ACS offices, courthouses, shelters, schools, hospitals, community partners, and SI PPS-identified high-risk locations.

This role is credential-flexible; duties assigned are based on licensure/certification. Staff may be hired under any approved pathway: CRPA, CASAC-T, CASAC, LMSW, LMHC, or LCSW.

DUTIES & RESPONSIBILITIES:

Outreach & Engagement

• Provide community-based outreach in ACS offices, courthouses, shelters, DSS/LDSS, SI PPS Hotspotting sites, hospitals, and other partner locations.
• Engage high-risk and underserved individuals using trauma-informed, harm-reduction, and motivational engagement approaches.
• Build rapport and deliver immediate, person-centered support in nontraditional settings.

Service Delivery (assigned based on credential level)

CRPA / Peer Services:

• Provide lived-experience-based recovery support, coaching, and relapse-prevention assistance.
• Facilitate or co-facilitate recovery, wellness, and peer groups.
• Navigate individuals to treatment, community services, and MAT providers.

CASAC-T / CASAC Services:

• Conduct SUD and Co-Occurring screenings, assessments, crisis interventions, and LOCADTR determinations.
• Provide individual and group counseling using MI/CBT and trauma-informed care.
• Assist with MAT engagement and coordination with medical partners.

LMSW / LMHC / LCSW Services:

• Provide clinical evaluations, psychotherapy, behavioral interventions, and crisis response.
• Conduct diagnostic assessments and create person-centered treatment plans.
• Link individuals to higher levels of care when medically/clinically indicated.

Care Coordination & Navigation

• Coordinate referrals for medical, psychiatric, MAT, housing, legal, social, and community services.
• Maintain active collaboration with ACS, SI PPS, shelters, EMS, law enforcement, hospitals, community-based organizations, and the court system.
• Provide warm handoffs and ensure closed-loop referrals using Camelot’s care-coordination platform.

Medication for Addiction Treatment (MAT) Support

• Provide education on low-threshold MAT and assist with same-day access pathways.
• Liaise with SIUH and Camelot’s medical partners to facilitate MAT initiation and follow-up.

Documentation & Compliance

• Complete documentation in the EHR in real time using agency-issued equipment.
• Submit OASAS Participant Encounter Forms for each meaningful contact in accordance with OASAS and Medicaid requirements.
• Maintain strict adherence to HIPAA and 42 CFR Part 2 confidentiality standards.

Professional Expectations

• Participate in supervision, team meetings, SI PPS learning collaboratives, and mandatory trainings.
• Maintain professionalism, cultural humility, and adherence to Camelot and OASAS policies.
• Represent Camelot appropriately within partner agencies and community sites.

KNOWLEDGE / SKILLS / QUALIFICATIONS:

• Any of the following credentials: CRPA, CASAC-T, CASAC, LMSW, LMHC, or LCSW
• High School Diploma/GED required for CRPA or CASAC-T; advanced degree as required by license.
• Experience working with individuals affected by substance use, trauma, homelessness, justice involvement, or co-occurring disorders preferred.
• Ability to work independently in field-based environments and maintain strong engagement skills.
• Knowledge of harm reduction, trauma-informed care, motivational interviewing, or community-based outreach.
• Strong computer literacy, including mobile documentation in an EHR.
• Ability to collaborate effectively with multidisciplinary and external partner agencies.

Pay: $40,000.00 - $65,000.00 per year
