Duties
- Conduct thorough patient assessments, including vital signs, mental status evaluations, and behavioral observations to develop individualized care plans.
- Administer medications and treatments as prescribed, including psychotropic medications, ensuring proper documentation in electronic health records (EHR) systems such as Epic or Cerner.
- Monitor patients during crisis episodes or acute episodes of mental health instability, utilizing de-escalation techniques and safety protocols.
- Collaborate with multidisciplinary teams—including psychiatrists, social workers, and case managers—to coordinate comprehensive care strategies.
- Educate patients and their families about mental health conditions, medication management, and coping strategies to promote ongoing recovery.
- Maintain compliance with HIPAA regulations and ensure confidentiality of patient information at all times.
- Assist in discharge planning by providing resources for community support services, including long-term care options like assisted living or memory care facilities.
Skills
- Proven experience in behavioral health nursing within hospital settings, outpatient clinics, or long-term care environments such as nursing homes or assisted living facilities.
- Strong knowledge of psychiatric medications administration and monitoring, including familiarity with ICD-10 coding for diagnoses.
- Skilled in crisis intervention techniques and trauma-informed care practices to manage acute behavioral episodes effectively.
- Proficiency with EMR/EHR systems such as Epic or Cerner for accurate medical documentation and patient record management.
- Ability to perform detailed patient assessments, including mental status exams and risk evaluations for self-harm or harm to others.
- Experience working with diverse populations, including children, geriatrics, individuals with developmental disabilities or dementia-related conditions like Alzheimer’s disease.
