Job Title: Internal Physical Therapy Traveler – Cambridge Healthcare ServicesReports To: Regional Director of Therapy / Facility Therapy ManagerFLSA Status: Non-ExemptPosition Summary:The Internal Physical Therapy Traveler provides high-quality, resident-centered physical therapy services across multiple skilled nursing facilities (SNFs) within the organization. This role ensures continuity of care, supports facility therapy teams, and contributes to optimal resident outcomes while maintaining compliance with professional and regulatory standards.Essential Duties and Responsibilities:Provide physical therapy services to residents in accordance with physician orders, facility protocols, and evidence-based practices.Conduct comprehensive resident evaluations, develop individualized care plans, and document progress accurately and timely in the electronic medical record.Deliver individual, group, and concurrent therapy sessions as appropriate to resident needs.Travel between assigned facilities on a regular schedule, adapting to each facility’s workflow and patient populations.Collaborate with facility leadership, therapy staff, nursing, and interdisciplinary teams to ensure high-quality resident care.Support, mentor, and train local therapy staff, promoting knowledge sharing and best practices.Participate in facility meetings, quality improvement initiatives, and continuing education opportunities.Maintain compliance with federal, state, and local regulations, including Medicare, Medicaid, and facility-specific policies.Required Qualifications:Licensed Physical Therapist (PT) in the state of California.Minimum 6 months of clinical experience in skilled nursing or post-acute care preferred.Strong knowledge of geriatric rehabilitation, mobility, and functional independence interventions.Excellent communication, collaboration, and problem-solving skills.Ability to travel regularly between facilities and adapt to diverse patient populations and environments.Competency in electronic medical records and documentation standards.Competencies:Clinical Excellence: Demonstrates strong clinical judgment and evidence-based practice.Flexibility & Adaptability: Adjusts effectively to new environments, patient needs, and facility workflows.Communication & Collaboration: Works effectively with interdisciplinary teams and communicates clearly with staff, residents, and families.Mentorship & Leadership: Supports and develops peers and local therapy staff.Continuous Improvement: Engages in ongoing learning and quality improvement initiatives.Compliance & Accountability: Maintains regulatory, ethical, and organizational standards.Key Performance Indicators (KPIs):Resident Satisfaction Scores / FeedbackTimely and Accurate Documentation ComplianceAchievement of Therapy Goals and Functional OutcomesNumber of Facilities Successfully Supported per ScheduleStaff Feedback on Mentorship and CollaborationParticipation in Quality Improvement and Professional Development ActivitiesPhysical Requirements:Ability to stand, walk, lift, and assist residents as required for patient care.Ability to safely transfer residents using appropriate techniques and equipment.Work Environment:Primarily within skilled nursing facilities, with frequent travel between multiple sites.Exposure to typical clinical settings and resident care activities.Compensation and Benefits:Competitive salary commensurate with experience.Travel stipend or mileage reimbursement as applicable.Comprehensive benefits package including health, dental, and vision insurance, retirement plan options, and paid time off.

Salary : $65 - $70
