Health Promotion Coach: Under the general supervision of
the Program Officer, the Health Promotion Coach, will provide
long-term services and supports care navigation and management
to high-risk community-dwelling clients (e.g., caregivers and their
loved ones). Health promotion coaches provide ongoing support
for the caregiver and at-risk client by creating additional access
points that address the changing caregiving and client needs. The
Coach develops and maintains a database for tracking and
facilitating ongoing navigation and linkages that continue to
educate, inform and support the caregiver and care partner
throughout their evolving care journey.
Example of Job Duties
- Become certified and proficient in the principles of Boston University’s CADER foundational
- Demonstrate flexibility in the delivery of care management and navigation services.
- Provides short-term, goal oriented, care management services that address the social

- Coordinate and refer clients to appropriate programs within the District, such as Adult Day
- Coordinate and refer appropriate clients in need of on-going well-being checks to the District’s
- Coordinate and refer caregivers in need of support services to the District’s Caregiver Center.
- Provide care navigation to clients and caregivers by referring those in need of outside support
- Conducts periodic assessment and documentation to measure the effectiveness of Care Plan
- Develops and maintains complete and accurate client files in compliance with best practices and
- Provide timely client data entry per departmental requirements, as well as preparing and
- Actively participate in Departmental Team meetings, as well as other committees as requested,
- Participate in the ongoing development of job knowledge relating to the growing body of
other related dementias, as well as other required trainings.
- Become certified in evidence-based programming offered at the District’s Caregiver Center (and
- Co-facilitates the District’s Caregiver Support Group.
- Provides care consultations by appointment or by walk-in
- Attends appropriate community meetings to represent District services
- Participates in presentations at community meetings to promote District services
- Provides assistance and delivery of the District’s Virtual Reality Program.
- Perform other duties as assigned.
Minimum Qualifications
• Education: Associate’s Degree minimum requirement but Bachelor’s Degree preferred in Sociology,
Public Health, Psychology, Counseling, Health Sciences, Human Services or other related field that
promotes the physical, psychosocial, and well-being of the persons being served.
- Experience: Experience working with older adults, those living with a form of dementia and/or
after hire). Knowledge of Caregiver stress and how it affects health. Ability to empathize. Must have
up-to-date knowledge of, and adherence to, applicable laws and regulations concerning
confidentiality, privacy, and protection of client health information. Excellent communication skills,
ability to build personal rapport, and keen understanding of the concept of “person-centered” care
and goals. Must be computer literate.
- Physical Conditions: sitting for prolonged periods; lifting and carrying not more than 30 lbs.;
operating assigned equipment. Ability to stand at least three (3) hours per day; able to sit at least
three (3) hours per day; climb stairs and physically able to travel.
- Mental Conditions: ability to work under pressure; ability to communicate tactfully and effectively
to adjust and be flexible to changing working conditions and job structure which may alter the
direction of daily schedule; and must be extremely organized.
- Other Requirements: Criminal records clearance. Must have own transportation for use on the job,
Compensation & Benefits
Hiring Range: $23.00-$25.00 per hour.
Work schedule : Full Time, 40 hours per week. Monday-Friday 8am-5:00pm
