Hospice Medical Social Worker (Per Diem)
Pay: $30/visit mileage & sick time
Territory: Temecula, Murrieta, Hemet, Riverside, Moreno Valley, Redlands, San Bernardino
We’re hiring two experienced Per Diem Medical Social Workers (MSWs) to join our growing hospice care team. If you’re a compassionate clinician who thrives in providing emotional support and advocacy for patients and families, we’d love to meet you.
What You’ll Do:
- Conduct psychosocial assessments and provide counseling to patients and families
- Collaborate with the interdisciplinary team to create and support individualized plans of care
- Connect families with community resources and provide end-of-life support
Requirements:
- Bachelor's Degree in Social Work (BSW) – required
- Minimum 1 year of healthcare experience within the last 3 years
- Valid driver’s license, reliable insured vehicle, and current CPR certification
