Job Title/Position: Referral/Intake
Reports To: DPCS/ Executive Director
JOB DESCRIPTION SUMMARY
Responsible for managing all aspects of the patient intake process. Establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
2. Ensures compliance with all state, federal, and ACHC referral/intake regulatory requirements.
3. Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
4. Establishes and maintains positive working relationships with current and potential referral sources.
5. Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
6. Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
7. Negotiates service pricing with insurance Case Managers and other payers within established financial and credit parameters.
8. Builds and monitors community and customer perceptions of Calstro Hospice Inc. as a high quality provider of services.
9. Gathers, collates, and reports referral statistics including key customer referral trends.
10. Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
11. Maintains comprehensive working knowledge of Calstro Hospice Inc. contractual relationships and ensures that patients are admitted according to contract provisions.
12. Participates in quality assessment performance improvement teams and activities.
13. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Calstro Hospice Inc.

POSITION QUALIFICATIONS
1. High School Diploma or equivalent required. Some college preferred.
2.At least one (1) year experience in health care management, preferably in home care operations.
3. Ability to market aggressively and deal tactfully with customers and the community.
4. Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practices.
5. Demonstrates good communications, negotiation, and public relations skills.
6. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
7. Spanish speaking preferred.

Pay: $23.00 - $25.00 per hour
