Medical Social Worker (MSW)
Job Type: Per Diem (Pay Per Visit)
Pay: $40– $60 per visit
Schedule: M-F availability
Territory: Orange County (Tustin, Newport, Lake Forest, Anaheim, Fullerton, etc.)
We’re looking for a compassionate Medical Social Worker / Social Work Associate to support patients and families in our hospice and palliative care program. This role provides psychosocial support, care planning, counseling, and community resource referrals while working closely with our interdisciplinary team.
What You’ll Do:
- Provide emotional and social support to patients and families
- Conduct assessments and develop care plans
- Connect patients with resources and community services
- Collaborate with nurses, physicians, and the care team
What We’re Looking For:
- MSW or BSW (hospice experience preferred)
- 1 year of social work experience (preferred)
- Strong communication and documentation skills
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
