Job Title/Position: Admission RN
Patient Care
Completes an initial assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
Provides professional nursing care by utilizing all elements of nursing process.
Assesses and evaluates patient’s status by:
- Writing and initiating plan of care
- Regularly evaluating patient and family/caregiver needs
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Additional Duties
- Participates in on-call duties as defined by the on-call policy.
- Ensures that arrangements for equipment and other necessary items and services
are available.
- Supervises ancillary personnel and delegates responsibilities when required.
Shift Requirements:
- Will be discussed during interview. Rotation of weekends are required.
Required Qualifications:
- At least 1 year experience in Hospice within that last 3 years
- Current California RN license
- Excellent written and verbal communication skills
- Valid CA Driver’s License and auto insurance
- Current CPR
- Current TB/Physical
- Pass Background Clearance
Required Qualifications:
- Bilingual (English/Spanish)
Preferred Qualifications:
- Baccalaureate degree in nursing
