Calm Waters Counseling is healthcare facility that focuses on helping people manage mental, emotional and physical stress through counseling, case management and CBRS services. Our goal is to help individuals discover freedom, be transformed, and take their lives and relationships to the next level!
We are looking for a Case Manager/CBRS provider to enhance the quality of client management and maximize satisfaction. The successful candidate will have an understanding of managing client cases.
Provider duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed out in the community.
Responsibilities
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
- Handle case assignments, draft treatment plans, review case progress and determine case closure
- Help clients achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help clients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relations and cooperate with medical team throughout the entire case management process
- Coordinate care with social services, health and governmental agencies
- Initiate regular interactions with clients to keep track of their progress and to ensure satisfaction
- Record case information, complete accurately all necessary forms and produce statistical reports
- Promote quality and cost-effective interventions and outcomes
- Assess and address motivational and psychosocial issues
- Adhere to professional standards as outlined by protocols, rules and regulations as addressed in the Social Work Code of Ethics
Skills
- Proven working experience as a case manager
- Excellent knowledge of case management principles
- Effective communication skills
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem solving skills and ability to multi-task
- Compassionate with teamwork skills
- Bachelor's degree in related healthcare/mental health field Required
