Description of Responsibilities
The PCC-Nursing Coordinator role requires strong relationships with the operations teams to act as a conduit to effective execution of new orders and implementation of custom customer solutions. This role is based in the corporate headquarters.

Responsibilities of the Patient Care Coordinator – Nursing Coordinator include the following:
- Collaborate with our internal Clinical Team to ensure that all required documents are in the patient chart before referring / contacting our contracted nursing agencies.
- Contact nursing agencies to coordinate medication administration appointments for our patients.
- Forward all patient referral documentation and confirm the patient administration appointment.
- Daily management of the nursing referral log.
- Receive all communications regarding nursing administration from patients, physician offices and nursing agencies.
- Triage of Nursing Agency complaints to the appropriate manager.
- Represent the pharmacy in a professional and courteous manner in all interactions with patients, providers, and peers.
- Work collaboratively with internal staff to ensure all issues are addressed in a timely manner.
- Report any misconduct, suspicious or unethical activities to the Compliance Officer.
- Additional tasks as required by management.
Qualifications and Preferred Skills
- 1 year of experience in a related field.
- Organizational skills sufficient to maintain consistently accurate records.
- Ability to evaluate options and to make efficient decisions.
- Computer skills that include proficiency with spreadsheets and use of the internet, as well as word processing, and efficient use of the internet and e-mail.
- Candidates should have a working knowledge of pharmacy applications.
- Knowledge of medical terminology
- Data entry skills required.
