ContinuumRx, the nation’s leading JV specialty and home infusion therapy expert, is seeking a Clinical Liaison (RN) to support our Birmgham, AL Team. The ideal candidate needs to possess strong interpersonal skills, have an energetic self-starter personality, and exhibits compassion in caring for people. The liaison will be responsible for introducing and educating clinically relevant providers on our products and services, generating referrals, and performing patient education and training on administration of therapy treatments. The liaison position also requires proactively building strong relationships with hospital case managers/social workers, physicians and clinical program directors within the assigned hospital(s) to grow referral rates and achieve service goals.
Roles & Responsibilities:
Include but are not limited to:- Responsible for introducing, educating hospital case managers, discharge planners, social workers, metabolic support teams, inpatient pharmacy, hospitalists, emergency room physicians and other hospital based clinically relevant providers on our products and services.
- Primary responsibilities will include entry of referral data, including patient demographic, diagnosis and medical history, physician, patient insurance plans, and drug/therapy order information, into the CPR system. The Liaison will be responsible for developing and maintaining effective relationships with our hospital partner referral sources, local pharmacy team and corporate intake team.
- Generating new referrals through consistent, coordinated sales and marketing activities within assigned facilities to promote generation of patient referrals.
- Establish professional/peer partnership relationships with referral sources (Discharge Planners, Case Managers, Home Health Agencies, Physician Office Nurse).
- Initial referral coordination
- Perform patient education and training on administration of their therapy treatment and use of associated equipment prior to discharge from the hospital.
- Perform infusion nursing care in the patient home (except where not permitted by state regulations) when the patient's home health agency or ContinuumRx Infusion Nursing staff are unavailable. If not licensed as a nurse (RN or LPN), will contact an available ContinuumRx nurse to perform the care.
- Coordinate patient treatment with Home Health Agency and other ancillary service providers
- Facilitate transfer of referred patients that will not be admitted to ContinuumRx to another home infusion provider if hospital requests.
- Ensure a smooth and complete transfer and proactively communicate with the patient and referral source throughout the process.
- Capture pertinent information about all patient transfers for analysis.
- Complete understanding of confidentiality with respect to Company proprietary information as well as information concerning patient/client confidentiality of protected health information (PHI) and electronic protected health information (EPHI); and following HIPAA guidelines regarding readily identifiable protected health information.
Qualifications and Experience:
1. Current, active and in good standing registered nurse license in the state of employment
2. 3 years nursing experience preferred, home infusion experience a plus.
3. Bachelor's Degree with a minimum of 5 years professional experience in healthcare preferred but not required.
4. Must be knowledgeable and experienced in all aspects of the specialty field
5. Must possess: teaching ability, leadership ability, pleasant personality and high professional standards for care. Must be well organized, be able to set priorities, work independently and must be effective team member.
6. Must be detail oriented and able to function efficiently in a fast paced work environment
7. Computer skills required (Windows, CPR , Office)
