California Impact Coalition Foundation is immediately hiring a Case Manager for our Transitional Housing Program for Non-Minor Dependents (THP-NMD).
We serve young adults ages 18–21 transitioning from foster care into independent adulthood. We’re looking for someone who is compassionate, organized, dependable, and passionate about helping young adults succeed.
This is a hands-on community-based position—not just a desk job. You’ll work directly with participants in their homes and communities, helping them build the skills and confidence needed to live independently.
What You’ll Do
- Manage an assigned caseload of young adults
- Meet with participants regularly in their homes and community
- Help participants achieve housing, education, employment, and independent-living goals
- Coach participants with budgeting, life skills, transportation, household management, and accessing community resources
- Conduct apartment/home visits and monitor health and safety
- Coordinate with county social workers, schools, employers, behavioral health providers, and community partners
- Complete case notes, progress reports, Needs & Services Plans, and required documentation
- Provide support and appropriate intervention during challenging situations
- Participate in meetings, trainings, and the on-call rotation as assigned

Experience in foster care, transitional-age youth, case management, housing programs, social services, behavioral health, probation, child welfare, or community-based programs is highly valued.
Transportation Requirements
Candidates must have a valid California driver’s license, current auto insurance, reliable transportation, and the ability to travel locally.
Mileage reimbursement is provided for approved work-related travel.
Compensation
$25.00–$30.00 per hour, depending on experience and qualifications.
