Home Care Nurse
Job Summary:
The home care nurse is a registered nurse who administers non-skilled nursing care to clients. The home care nurse serves as a liaison nurse between the medical community and the agency initiating, establishing and evaluating homecare services to clients. These services are performed in accordance with the program guidelines for the program to which each client is enrolled.
Essential Functions:
1. Conducts the admission of the client to Caldwell Home Services, Inc by:
a) Obtains a client/client history (medical as appropriate to services), particularly as it relates to the present condition, from the client and/or family member(s).
b) Conducts an assessment of the client/client as related to the services required.
c) Evaluates the client's condition to determine what assistance will be available from family members in caring for the client.
d) Evaluates the client, family member(s) and home situation to determine if additional community services are appropriate.
e) Evaluates the client's condition and home situation to determine if the services of a PCA/CNA will be required and the frequency of this service.
f) Interprets nursing and other services of the Agency to clients and families as a part of planning for care.
g) Explains any necessary precautions associated with treatment.
h) Obtaining client and/or responsible party signatures on necessary forms.
2. Develops and implements the client's care plan.
3. Provides non-skilled nursing care as outlined in the care plan to include the following:
a) Teaches, supervises and counsels the client and family members regarding the care needs or other related problems of the client at home.
4. Is responsible for the care given by the Home Care Aide by:
a) Provides the Home Care Aide with a written plan of care/task sheet and specific duties the Aide is to perform for each individual client.
b) Supervises and evaluates the care given by the Home Care Aide as needed and at a frequency consistent with company policy.
c) Participates in periodic conferences with Administration concerning the Home Care Aide's performance.
5. Documents findings and services rendered to the client, conferences held with the healthcare team or community resources, changes noted in the client's condition and/or family and home situation, makes revisions in the nursing care plan as needed, records Supervisory visits conducted with the Home Care Aide, evaluates client care and progress and closes charts of discharged clients.
6. Evaluates the effectiveness of homecare services rendered to the individual and family.
7. Informs the client and/or responsible party of any change in the plan of care
8. Reports to the Administrator giving a summary of the day's work.
a) Maintains a daily timesheet outlining the clients/clients served, not at home, etc. and mileage
b) Discusses with the Administrator problems concerning the clients and how these may best be resolved.
c) Discusses with the Administrator changes in client needs and how these can best be met.
d) Discusses with the Administrator the need for involvement of other members of the home care team.
10. Coordinates total client care.
11. Cooperates with other agencies providing services to provide continuity of care and to implement a comprehensive care plan.
12. Participates in client care conferences and record audits.
13. Participates in the planning, operation and evaluation of the nursing service.
14. Provides care and follows agency procedures and policies.
15. Is familiar with service outlines of the PCS, CAP/_DA, CAP/_C, Program for All Inclusive Care for Elderly (PACE), and Veteran in-home aide policies and procedures.
16. Performs other duties as necessary.
17. Participates in performance improvement activities.
18. Attends continuing education as required by company policy and licensure.
19. Complies with agency Infection Control policies.
20. Has experience, training and shows demonstrated competency in specialty service criteria as outlined in the organization's service manual.
21. Conducts staff orientation in accordance with policy and procedure.
