Job Overview
CABS Health Network is seeking a highly organized and community-focused Housing Services Advocate to support its expanding portfolio of home-based environmental, safety, and housing stabilization services. This individual will operate at the Patient Navigator level for home remediation services but will also serve a more advanced role when supporting tenancy adequacy and housing access services.
In those cases, the Assistant will be expected to provide direct tenancy support services, not just navigation or coordination. Therefore, candidates must possess the skills and experience necessary to deliver hands-on support to participants, especially in navigating housing systems and sustaining tenancy.
Key Responsibilities:
Client Navigation & Service Coordination
- Serve as the primary point of contact for clients receiving home remediation, accessibility, safety, or tenancy-related services.
- Coordinate with vendors and contractors to schedule assessments, installations, and remediation work.
- Confirm appointment windows with clients and ensure client availability and preparation for service delivery.
Tenancy Support Services
- Assist clients in searching for housing, completing rental applications, and preparing for lease agreements and interviews.
- Facilitate move-in transitions, including coordination of furnishings, pantry items, and environmental readiness based on the member's needs.
- Support utility setup and payment assistance including activation, back-payments, and up to six months of support.
- Provide tenancy-sustaining services including landlord mediation, eviction prevention, and support for housing subsidy renewals.
- Pre- and Post-Service Follow-Up
- Conduct structured post-job completion surveys to evaluate client satisfaction.
Program Tracking & Documentation
- Maintain accurate logs of completed work, open cases, client complaints, and vendor follow-through.
- Support intake and eligibility screening processes by collecting key documents and logging service needs.
- Track progress on a range of service categories, including but not limited to:
- Home remediation and safety improvements
- Accessibility modifications
- Utility activation fees
- Rental assistance and housing transition
- Pre-tenancy and tenancy-sustaining services
- Community-based transition and navigation supports
Outreach & Engagement
- Reach out to referred or inactive clients to educate them on available services and support re-engagement.
- Assist with client education around housing rights, responsibilities, and available remediation supports.
- Coordinate closely with case managers, social workers, and external vendors.
Minimum Qualifications
- Associate Degree in Human Services, Public Health, Social Work, or a related field highly desireable).
- At least 4 years of experience in client-facing roles such as Patient Navigator, Housing Specialist, or Field Coordinator.
- Minimum 3 years of experience providing direct tenancy support services.
- Demonstrated ability to manage multiple service areas and coordinate with field contractors.
- Excellent interpersonal, documentation, and follow-up skills.
- Familiarity with Medicaid services, housing navigation, or NYC social service programs preferred.
- Proficient in Microsoft Office (Excel, Word) and data tracking systems.
- Bilingual abilities (e.g., Spanish, Haitian Creole, Russian, Mandarin) are highly desirable.
- Perform 30-day and 90-day follow-up calls to assess if services remain effective.
- Escalate unresolved issues or concerns to the Home Remediation Supervisor.
