- Provide individualized education and emotional support
- Coordinate multidisciplinary care and appointments
- Identify and address barriers to care
- connect patients wiht community and financial resources
- Facilitate communication between patients, families, and care teams
- Track patients throughout their care continuum to prevent care gaps.
