What are the responsibilities and job description for the Paraprofessional position at C4Y Residential Facility?
Client Supervision and Support:
The Paraprofessional provides direct care and support services to adolescents in a therapeutic level III group home environment. Under the supervision of the Qualified Professional (QP) and House Manager, the Paraprofessional assists with the implementation of individualized treatment plans, teaches daily living skills, and ensures the health, safety, and well-being of all residents. This role is essential in supporting a positive, structured, and therapeutic environment that promotes client growth and development.
Essential Duties and Responsibilities:
- Provide supervision and guidance to residents in daily living activities, ensuring safety at all times.
- Assist clients with personal care, hygiene, meal preparation, housekeeping, and independent living skills.
- Support clients in following their daily schedule and treatment plans as directed by clinical staff.
Behavioral Support:
- Implement behavior support plans, de-escalation techniques, and crisis intervention strategies under the direction of the QP or House Manager.
- Monitor and document client behaviors, mood, and participation in activities.
Activity Facilitation:
- Engage clients in recreational, educational, and social activities that promote skill development and positive peer interaction.
- Assist in planning and supervising community outings and events.
Documentation:
- Complete required documentation such as daily progress notes, behavior logs, incident reports, and shift reports accurately and timely.
- Maintain confidentiality and protect client information in accordance with HIPAA and agency policies.
Team Collaboration:
- Participate in staff meetings, in-service training, and supervision sessions.
- Communicate observations and concerns regarding client progress, behavior, or needs to the House Manager or QP.
Health and Safety:
- Maintain a clean, safe, and organized environment for all residents.
- Ensure the proper storage, distribution, and documentation of client medications if trained and authorized to do so.
- Respond to emergencies, crises, or incidents promptly and appropriately.
Other Duties:
- Provide transportation for clients to appointments, school, or community activities as assigned.
- Perform other related duties as required by the House Manager, QP, or Program Director.
Minimum Qualifications:
Education:
High School Diploma or GED required. Associate’s degree in Human Services or related field preferred.
Experience:
- At least one (1) year of experience working with adolescents in a behavioral health, group home, or similar setting.
- Experience providing direct care services to individuals with mental health or developmental disabilities is desirable.
Credentials:
- Must possess a valid driver’s license and an acceptable driving record.
- Must successfully pass criminal background checks and other required pre-employment screenings.
- Completion of all required training CPR/ First Aid, BBP, CPI & Med Administration within 30 days of hire.

- Basic understanding of behavioral health and adolescent developmental needs.
- Ability to maintain professional boundaries and model appropriate behavior.
- Effective communication, problem-solving, and teamwork skills.
- Ability to remain calm and effective in crisis situations.

Work Environment:
- Residential group home level III setting. Secured and structured setting!
- Requires working hours: Weekdays 2pm - 9pm, Weekends 10am - 10pm, Holidays, and possible on-call shifts. Rotating weekends!
- Exposure to potentially challenging behaviors or emotionally intense situations.
Physical Requirements:
- Ability to lift up to 25 lbs., stand, walk, bend, and participate in physical interventions as needed.
- Ability to participate in client recreational and community activities.
Job Types: Full-time, Part-time
Pay: $17.00 per hour
