C3 Cares is seeking a compassionate, community-focused, and innovative Family Nurse Practitioner (FNP) to join our team full-time. We are redefining how healthcare is delivered by bringing primary care services directly to underserved communities through our place-based clinics in public housing communities. This position offers the opportunity to make a meaningful impact on health equity by providing comprehensive, patient-centered care to children and adults in under-resourced neighborhoods.
Position Summary:
The FNP will provide high-quality, comprehensive clinical evaluation, education, and management for patients across the lifespan (children and adults) for preventive, acute, and chronic conditions. Services will be delivered onsite at clinics, via telehealth, and during home visits. The FNP will collaborate with a multidisciplinary care team to coordinate medical and social services, address social determinants of health, and engage patients in their care to improve outcomes.
Primary Responsibilities:
● Provide comprehensive primary care services for children and adults, including assessment, diagnosis, treatment, and chronic disease management.
● Deliver preventive care services such as screenings, immunizations, and health education.
● Collaborate with a multidisciplinary team to address social determinants of health and coordinate care.
● Support patient engagement, education, and empowerment to promote health equity.
● Document care in the electronic health record and participate in quality improvement initiatives.

Qualifications:● Master's or Doctorate degree in Nursing (MSN, DNP, or equivalent).
● Active and unrestricted DC Family Nurse Practitioner license.
● Active ANCC or AANP certification.
● 2-5 years of clinical experience as a Nurse Practitioner preferred.
● Experience working with Medicaid, low-income, or under-resourced communities strongly preferred.
● Experience in public health or community health settings highly desirable.
● Proficiency in EHR systems.
● Commitment to promoting health equity and addressing healthcare disparities.
Location & Schedule:● Full-time position (40 hours per week)
● Based at C3 Cares place-based primary care clinics located within public housing communities in Washington, D.C.
